Instructions:
Please use the blank space on the memory book form to share your memories, stories, adventures, travels, or reminisce about your days at UMass. Type or neatly write all information clearly and be sure to keep all text inside the page borders, which will ensure enough space to bind the book together without loosing any text. DO NOT WRITE ON THE BACK OF THE PAGE OR ADD ADDITIONAL PAGES.

Your Memory Book page will be printed as you submit it, so make sure the information on the form is correct. Please remember to mail your memory book form, photo (if applicable), completed reply card and payment to the Alumni Association in the enclosed labeled envelope you received in your packet postmarked no later than February 25th.

*Please note that Memory Books will only be printed and distributed to those who have paid in advance*

*Memory Book Packets were mailed to all 1958 Classmates with good addresses in our database.
